The Alliance

Built for the girls who weren't told.

PCOS Alliance is a women's health nonprofit organization preventing the progression of PCOS/PMOS and onsets of metabolic syndrome in girls and women through early intervention, integrative care resources, and trauma-informed support.

Eight years undiagnosed. That is why whole-person care is method here, not philosophy.

Inika Mehra is the founder of PCOS Alliance, recognized for her impact and advocacy in women's health. Inspired by her own journey with PCOS/PMOS, she started the Alliance to make the resources that gave her back her confidence and her health accessible to everyone.

She also held the title of Miss San Francisco 2025, the first South Asian woman to earn the title in Miss America's 99-year history, using the platform to amplify PCOS/PMOS awareness and raise more than $10,000 toward women's health initiatives.

In 2025 she won both Young Woman of the Year and Leader of the Year at the Women Changing the World Awards, and holds a President's Volunteer Service Award. She has spoken at institutions and conferences including GirlCon Tech Conference, UCSF, Stanford Medicine, the Association for Women in Science, and the Rotary Club of San Francisco.

A patient perspective

Eight years, and no one connected the dots.

Inika Mehra had symptoms at eight and a diagnosis at sixteen. What happened in between is the reason the Alliance exists.

This account touches on years of dismissed symptoms and delayed care.

Puberty arrived early, ahead of her peers. Then came rapid, unexplained physical changes and weight gain, and excess terminal hair growth — a sign of hyperandrogenism, though nobody used that word at the time. She was eight years old. There was an initial screening. It returned no diagnosis and no guidance, and she was told she did not have PCOS/PMOS.

Each symptom got its own doctor

From age eleven

Severe hirsutism began at eleven. Her periods were irregular enough to cause severe anemia. Cystic acne, then hair loss. Chronic fatigue, migraines, pain behind the eyes. Every one of these was treated on its own terms, by whichever specialist it seemed to belong to, and no one drew a line between them.

A name, but not a plan

Diagnosis at sixteen

The diagnosis came in a brief consultation with little discussion around it. Two options were offered, birth control or spironolactone, and she chose spironolactone. There was no comprehensive care plan and not much counseling to go with it. On spironolactone alone, the symptoms did not improve.

A diagnosis without a plan is just a word for what is already happening to you.

So she went looking

The quest

She read everything she could find and began trying integrative modalities, and slowly understood that the diagnosis went deeper than its surface. Nutrition, lifestyle, stress management and nervous-system regulation turned out to be pivotal rather than peripheral. Unaddressed trauma, she found, sits underneath the symptoms and needs a healing pathway of its own.

What actually worked

Carrying it forward

Neither approach on its own. A blended Western and Eastern model produced better outcomes than either could alone: conventional medicine managing the acute symptoms, holistic tools carrying emotional well-being, cycle regulation and daily life. Comprehensive care, lifestyle medicine, nutrition and mental health as companions to medical treatment, never as substitutes for it.

That model is what the Alliance now teaches. It took her thirteen years to assemble. It should take a girl one workshop.

Where it led

The model that came out of those years is the model the Alliance teaches: conventional care for acute symptoms, holistic tools for lasting balance, and mental health treated as essential — not optional.

Nine signals worth writing down

If your cycle, skin, energy, or mood has been telling you something for a while, this is how to turn that feeling into notes a clinician can work with.

Most girls are told their symptoms are ordinary. Cramps are ordinary. Acne is ordinary. Being tired is what happens when you are sixteen and busy. And often that is true — which is exactly why the pattern matters more than any single sign.

Start with the pattern, not the symptom

A cycle that arrives whenever it likes, hair that thins at the crown while it grows on your chin, weight that climbs without a change in habits, an afternoon crash you can set a clock by — separately, each has a dozen explanations. Together, they are a picture worth showing someone.

Write down what you notice for two months. Dates, not adjectives — a note on your phone is enough.

What to bring to the appointment

Insulin is usually part of the story

PCOS/PMOS is as metabolic as it is hormonal. When cells stop responding well to insulin, the body makes more of it, and higher insulin nudges the ovaries toward more androgens — the hormones behind the hair and skin changes. That loop is also the reason prevention works: what steadies blood sugar tends to steady the rest.

You are not managing a character flaw. You are managing a signalling system — and it responds.

What helps, starting this week

Protein and fibre at breakfast rather than sugar alone. Movement you would do again tomorrow — a walk after dinner counts. Sleep defended like an appointment. None of this is a cure, and none of it is a punishment; it is the daily version of the same care a clinic gives you.

The care grant

The care that works is the care she can't afford.

Apply for a grant

Our micro-grants fund essential PCOS/PMOS treatments insurance won't cover: $200 to $400, awarded on need, paid straight to the provider.

Seed funding

Community funded, every dollar to care.

The Rotary Club of San Francisco funded the first round of micro-grants. Nothing is held back for overhead: awards are paid directly to providers.
